Office Policies

Payment Policy:

Each patient is responsible for their own account on the date of the treatment. The fees are discussed prior to the treatment being done. This is why we offer free no obligation consultation so that you can understand the proposed treatment plan. If you require extensive treatment and cannot afford to pay it at once, we offer flexible payment plans.

Clients with dental insurance plans are asked to bring their identification card at each visit. If there are any changes within your insurance policy or address change, you must let us know. It is still your responsibility to pay for your treatment on the day of treatment. We are more than happy to send the claim to your insurance company on your behalf and you will receive the payment either directly deposited to your account or via cheque by mail. It is your responsibility to know your coverage. If there is extensive work that needs to be done, we are more than happy to assist you in submitting a predetermination claim, so you will know exactly how much your plan will cover and how much you will need to pay out of your pocket.

Payments can be done by Cash, Debit, Visa, Master Card, cheque and more. (Valid credit card needed on file if using cheque)

Privacy Policy:

Radiance Dental Hygiene strictly follows and enforces the PHIPA policy. PHIPA is short for
Personal Health Information Protection Act.

All new patients will be asked to read, understand and sign our privacy policy.

Your personal and health information will be kept private within the office and protected by the privacy officer, Samer Issawi. Your personal and health information is kept confidential within the office and can be only disclosed under certain circumstances such as your dental insurance, investigations related to law enforcement, national security, international affairs, and emergencies. If a referral is needed to another specialist, they may require photos or digital X-rays. Your understood verbal consent is necessary prior to sending any records or documents.

Safety Policy:

To ensure the safety of all our patients, a thorough medical/dental history is taken at the initial consultation appointment and is updated at each follow-up appointment.

Medical / Health Changes Policy:

If there are any health changes since your last visit you must let us know, especially those who have been hospitalized for any reason. It is very important to know if your family doctor or a specialist have prescribed something new or told you that you will need premeditations for your dental visits.

We are trained in First Aid, CPR & Defibrillation: (CPR = Cardiopulmonary Resuscitation).
Training is completed every year to keep up with the latest standards and recommendations of CPR. Defibrillation is the definitive treatment for the life-threatening cardiac arrhythmias, ventricular fibrillation and pulseless ventricular tachycardia.

Infection Prevention Policy:

Our office follows the Universal Infection Control Protocol.

This protocol was designed to minimize exposure and transmission of a wide variety of
micro-organisms. Since every patient poses a potential infection risk we use standard
precautions for all patients at all times.

All areas that are touched by a clinician are protected by plastic barriers, which get disposed of after every patient. All surfaces that were covered by plastic barriers are being wiped by Optim 33 TB disinfectant. Norwalk-like viruses (Feline Calicivirus), the Polio virus and even the common cold virus (Rhinovirus) are resistant to many surface disinfectants. But OPTIM 33 TB Surface Cleaner & Intermediate Level Disinfectant kills them all in one minute. It also has a one minute claim against: Mycobacterium terrae, Human Coronavirus (SARS), HIV-1, Hepatitis B, Hepatitis C, Avian Influenza (Bird Flu), Salmonella, E.Coli, Pseudomonas, Staphylococcus aureus MRSA and other viruses & bacteria*, and can achieve a 99.999% broad-spectrum kill of vegetative bacteria in 30 seconds!

We use many disposable items to minimize the unlikely risk of infection. The instruments that are being used for dental procedures are being soaked in an ultrasonic bath for 15 minutes in a non-ammoniated ultrasonic cleaner solution. Than it is rinsed thoroughly and is ready to go into a sterilization stage by Autoclave Machine for 15 minutes.

Retainer Repair & Replacement Policy

Bonded Wire Retainer
If a bonded retainer comes loose within 6 months after being bonded, we will repair or replace it free of charge depending on certain circumstances. If the retainer requires repair or replacement after the 6 month period there will be a fee.

Essex Removable invisible Retainer
If your removable retainer breaks within the first 6 months after given to you, we will replace it once for free. There will be charges for any future replacements.
All lost removable retainers will be replaced at the normal fee.
